Abstract

|
목적: 본 연구는 소아 환자 분류도구(PPCS)를 개발하고 신뢰도와 타당도를 검증하기 위해 수행되었다.
방법: 연구는 다양한 병동을 포함한 어린이병원에서 수행되었다. 소아 환자 분류도구의 내용 타당도를 검증하기 위해 전문가 집단에 의한 델파이 기법을 적용하였고, 7개 병동을 대상으로 7명의 간호관리자와 총 29명의 간호사가 환자분류를 시행한 216명의 자료로 급내 상관계수를 구하여 조사자 간 신뢰도를 분석하였다. 구성타당도 검증은 환자분류 조사 당일의 환자 집단의 나이, 재원일수, 재원 형태(재원 중, 응급실 입원, 외래 입원, 퇴원), 진료과 등을 집단 비교법을 이용하여 검증하였다.
결과: 개발된 소아 환자 분류도구는 12개 영역, 55개 간호활동, 80개 항목과 적용지침으로 구성되었다. 조사자 간 신뢰도는 일치도가 높은 것으로 나타났으며 (r=.90), 연령과 재원일수, 재원 형태와 진료과별 환자분류점수가 통계적으로 유의한 차이가 있는 것으로 확인되어 구성타당도가 검증되었다 (p<.05). 최종 PPCS 환자분류군은 4개군으로 분류되었다.
결론: 본 연구를 통해 개발된 PPCS는 소아 환자의 투약 및 상대적 복합성에 대한 간호 요구도가 반영된 유용한 환자분류 도구로 사용될 수 있을 것이다.

Purpose: This study was performed to develop a valid and reliable Pediatric Patient Classification System (PPCS).
Methods: The study was conducted in a children’s hospital which included various ward settings. Content validity was analyzed by Delphi method and to verify intraclass correlation reliability, 7 nurse managers and 29 staff nurses classified 216 patients. To verify construct validity, the staff nurses classified 216 patients according to PPCS comparing differences by age, days of stay, type of stay and medical department.
Results: The developed PPCS has 12 categories, 55 nursing activities and 80 criterions. High agreement among nurses (r=.90) suggested substantial reliability. Construct validity was verified by comparing differences in age, days of stay, type of stay and medical department (p<.05). The entire patient group were classified to four groups using PPCS.
Conclusion: The findings suggest that PPCS would be a useful tool for estimating nursing demands related to medications and the complexity of pediatric patients.
